When the patient turns 12 years old, proxy access automatically changes to limited access to align with Minnesota state law.
Full proxy access for ages 12 to 17 requires the patient to meet with the provider, and both must sign the MyChart Proxy Request Form; access expires at 18.

Adult patients can grant full proxy access through the Sharing Hub, by signing the MyChart Proxy Request Form, or by contacting MyChart Support.
Adult patients may use the Sharing Hub in MyChart to grant full proxy access with other adults.
