1. Parents or guardians can request full proxy access for children 0 to 13; parents of patients 14 to 17 get limited access.

   Patient parents/guardians can request full proxy access for a child between the ages of 0-13 years old. Parents/Guardians of patients 14-17 will have limited access

2. Child (0 to 13) and adult to adult proxy requests can be made through an online form in MyChart or a downloadable form.

   This can be done in MyChart using an online form, or the form can be downloaded, completed and provided to your provider

3. Teens need parent or guardian permission first before teen proxy can be granted.

   Teen patients will need permission from the parent or legal guardian FIRST before Teen Proxy can be granted.

4. Each person requesting access must complete a separate form.

   A form needs to be completed for each person requesting access.
